Terms & Conditions
These terms cover the dietetic services provided by The Clinical Plate and your use of this website. By booking an appointment or sending an enquiry, you agree to them.

1. About this practice
The Clinical Plate is the private dietetic practice of Qaswara Affaf, a New Zealand Registered Dietitian (NZRD) based in Wellington. Services are provided in person in Wellington Central and by secure video consultation throughout New Zealand.

2. Scope of service
Dietetic consultations provide individualised, evidence-based nutrition assessment, advice and support. They work alongside — and do not replace — the care of your GP, specialist or other treating clinicians.
- Services are delivered within the scope of practice of a New Zealand Registered Dietitian.
- Where your needs fall outside that scope, or would be better met elsewhere, you will be told and referred appropriately.
- Medication is never prescribed, started, stopped or adjusted; changes to medication are a matter for your prescriber.
- Nutrition care supports health outcomes but cannot guarantee them. No specific clinical, symptom or weight result is promised.
3. Bookings
Appointments can be requested through the booking page. Submitting a request is not a confirmed appointment: a time is confirmed only once you receive a confirmation email. No referral is required.
You are responsible for providing accurate information about your health, medications and supplements. Advice is based on what you disclose, and incomplete information may affect how appropriate that advice is.
4. Fees and payment
- All fees are in New Zealand dollars and include GST. Current prices are shown on the services and booking pages.
- Payment is due before or at the time of your appointment, by secure payment link or online bank transfer.
- Card details are never collected through this website.
- An itemised receipt is provided for every consultation, including the provider details insurers require.
- Fees may change from time to time. The price confirmed at the time of booking is the price you pay for that appointment.
Health insurance: many New Zealand insurers reimburse registered dietitian consultations under specialist or wellness benefits. Cover is a matter between you and your insurer — please confirm your entitlement before booking. You pay the practice directly and claim reimbursement yourself.
5. Cancellations and missed appointments
- Please give at least 24 hours' notice to cancel or reschedule. There is no charge for changes made with that much notice.
- Cancellations made less than 24 hours before the appointment, and appointments not attended, may be charged the full consultation fee.
- Arriving late shortens the appointment rather than extending it, as the following appointment cannot be delayed.
- Where an appointment has to be cancelled by the practice, it will be rescheduled at the earliest convenient time, or refunded in full if already paid.
- Fees may be waived at the practitioner's discretion in genuinely exceptional circumstances.
Package and programme fees cover the appointments described at the time of purchase. Unused appointments within a package are valid for twelve months from the date of purchase.
6. Online consultations
Video consultations are conducted over a secure platform, and a link is sent with your confirmation. You are responsible for having a suitable device, a stable internet connection and a private space. If technical problems prevent a consultation from going ahead, it will be rescheduled at no additional cost.
7. Your information
Information you provide is handled in accordance with the New Zealand Privacy Act 2020 and the Health Information Privacy Code 2020. See the privacy policy for what is collected, why, and how to access or correct it.
8. Website content and intellectual property
All content on this website — including text, articles, imagery, resources and the brand marks — is owned by The Clinical Plate or used under licence. You may read, print and share it for your own personal, non-commercial use. Republishing, reselling or adapting it commercially requires written permission.
Meal plans, handouts and resources provided as part of a consultation are prepared for you individually and are not intended for distribution to others.
9. Client stories and reviews
Any client reviews or stories published on this website are shared with the client’s written consent and are individual experiences. Outcomes vary between people, and nothing shown should be read as an indication of the results you will personally achieve.
10. Liability
Nutrition advice is provided in good faith and based on the information available at the time. To the extent permitted by law, The Clinical Plate is not liable for indirect or consequential loss arising from use of this website or from general information published on it.
Nothing in these terms limits your rights under the Consumer Guarantees Act 1993 or the Fair Trading Act 1986 where those Acts apply.
This website may link to external sites. Those sites are not controlled by The Clinical Plate and no responsibility is taken for their content.
11. Changes to these terms
These terms may be updated from time to time. The version published on this page at the time you book applies to that booking. The date at the top of this page shows when it was last revised.
12. Governing law
These terms are governed by New Zealand law, and the New Zealand courts have exclusive jurisdiction over any dispute arising from them.
13. Concerns and complaints
If you are unhappy with any aspect of your care, please raise it directly with Qaswara at qaswara@theclinicalplate.com so it can be addressed. You also have the right to complain to the Health and Disability Commissioner, and rights under the Code of Health and Disability Services Consumers’ Rights.
